(YANKTON, SD) – Horizon Health Foundation recently received a donation from Yankton Community Foundation for the purchase of a Connex Spot Monitor, also known as a “Nurse on Wheels,” for patients at the Yankton Community Health Center.
A Nurse on Wheels provides accurate vital signs measurement, including blood pressure averaging, digital respiration rate, spot checking, interval monitoring and custom scoring across patient populations. The secure medical device allows fast access to vitals while protecting patient privacy, data integrity and compliance.
“The Nurse on Wheels provides an efficient and complete set of patient vitals within 15 seconds, which helps in staff workflow efficiencies, provides better diagnosis accuracy and reduces vitals documentation errors,” said Misty Rudebusch, medical director at Horizon Health Care. “Our goal is to get patient vitals taken so we can move onto their health care needs and any care coordination that is needed in a timely fashion.”
With the Yankton Community Foundation’s gift, Horizon Health Care’s Yankton clinic can continue to provide quality care to all its patients, regardless of ability to pay.

Horizon Health Foundation, a tax-exempt, non-profit corporation, seeks through responsible activities to encourage and derive sustaining gift support from current and former patients and interested friends, including corporate entities. The Horizon Health Foundation, operating in coordination with Horizon Health Care, is the duly designated fundraising arm for Horizon Health Care. In the broadest sense, the purpose of the Foundation is to create an awareness of the financial needs of Horizon Health Care, which are not provided by patient charges and to systematically implement plans by which dollar needs can be met.
