(FAITH, SD) – Horizon Health Foundation has received a $500 donation from the Faith Area Community Foundation to support the Faith Community Health Center.
The donation will fund the purchase of nine digital home monitoring blood pressure arm cuffs. The cuffs are for uncontrolled hypertensive patients with financial need. They will help patients keep track of their daily blood pressure readings. If the blood pressure reading indicates a level at which further intervention is needed, the patient receives an alert to call for help.
“The effective management of hypertension can prevent cardiovascular diseases, lower overall health care costs, and improve the general well-being for the patient,” said Rachel Olugbenga, FNP-BC, DNP, and provider at Faith CHC.
Faith Community Health Center sees patients from the community of Faith and the greater Meade and Ziebach County area.
With the gift from the Faith Area Community Foundation, Horizon Health Care’s Faith CHC can continue to provide quality care to all its patients, regardless of ability to pay.
The Horizon Health Foundation was established in 2016 to ensure rural health care access to those in need and provide funding for Horizon Health Care.
